To file the request, write an email to archivo@barraquer.com.co attaching the required documents according to the following recommendations, in addition to the dates of specific attention requested or, failing that, if you require complete information.
Please attach:
- Photocopy of legible identification document.
- Duly signed request letter for the clinical history and/or diagnostic images, authorizing the sending of the information to the email you refer.
Please attach:
- Photocopy of the legible identification document of the authorized person.
- Photocopy of the legible identification document of the patient.
- Letter of Request for medical history and/or diagnostic images duly signed by the authorized person and the patient, authorizing the sending of the information to the email address you refer.
Please attach:
- Photocopy of the legible identification document of the father, mother or legal representative.
- Photocopy of the legible identification document of the minor.
- Document that proves you as father, mother or legal representative (civil registry of birth or document that proves you as legal representative)
- Letter of Request for medical history and/or diagnostic images duly signed, authorizing the sending of the information to the email you refer.
